General Grossing Guidelines: The purpose of the grossing is to describe all gross changes, and take all required sections for documentation of the type of disease, it's extent, and to evaluate margins of resection. In general, it is more important to thoroughly evaluate the specimen, than to keep the specimen intact. To that end, the solid specimens should be sectioned at every one cm. or less to reveal any hidden findings. Ink should be used to identify the margins. When in doubt, ink the margins, as extra ink does not do any harm. Different colored inks may be used when there is a probability that the two margins may be confused with each other in tissue sections. Sections should be no more than 2 mm thick. Hemorrhagic and adipose tissues may be fixed overnight to obtain better sections. Usually one section per cm. of the tumor is recommended. However, fewer sections may be obtained from such routine specimens as well circumscribed uterine leiomyomata. Cutting with a movement of the knife, rather than simply use of pressure, would yield better section. A knowledge of how various tumors are staged is important in guiding you regarding the sections that need to be taken to accurately stage tumors.
